Seeking a Community Advisory Board for GLIDE Wellness Center!

We want your input!  The Wellness Center at GLIDE Health Services  is developing a Community Advisory Board (CAB) for members to offer constructive suggestions to help improve, maintain and expand clinical services here at GLIDE.

What? The CAB will consist of 11 members, 6 of which will be clients, so it is a client majority board.

Who? Hopefully, you, if you are an active consumer of GHS.

When? The board will meet 4 times per year (quarterly) January – October for 1.5 hours in the afternoon.  The exact day is to be determined.

Where? Glide, 330 Ellis St, unless otherwise indicated.

How? Please contact Alyse Berry no later than Wednesday, January 5th at (415) 674-5188 for more information.  We will be sending out applications the first week of January and will be scheduling interviews in mid to late January.
